A Case of Tubulovillous Adenoma of the Ampulla of Vater in Asymptomatic Adult

Abstract

Duodenal adenomas arising from the ampulla of Vater is characterized by uncommon lesion that are often asymptomatic. They have a high risk of malignant development, similar to the adenoma-to-carcinoma sequence that occurs elsewhere in the gastrointestinal tract. The traditional management of ampullary tumor is a surgical approach such as pancreaticoduodenectomy and ampullectomy. However, such surgical treatment may have a high rate of operative mortality, morbidity, and complications. In addition, it may lead to increased medical costs, surgical scars, and psychological distress. With the recent improvements in endoscopic devices and techniques, if ampullary tumor is diagnosed early, an endoscopic resection can be used as an alternative option. Thus, early detection of ampullary adenoma is of utmost importance. The authors report one case of tubulovillous adenoma of ampulla of Vater of the duodenum in an asymptomatic adult with review of several previous studies.

십이지장 팽대부에 발생하는 선종은 상부위장관에 발생 하는 종양 중 드문 병변이며, 대부분 무증상이다. 이러한 팽대부 선종은 다른 위장관과 마찬가지로 선종-암화과정을 통해서 암으로 발전할 수 있는 전암성 병변이다. 팽대부에 발생하는 종양에 대한 전통적인 치료방법은 개복을 통한 췌십이지장절제술 또는 팽대부절제술 같은 외과적 수술이었으나, 이러한 수술적 치료는 수술 중 사망의 가능성이 높고, 회복기간도 길며, 수술 후 합병증의 발생 가능성도 높은 단점이 있었다. 하지만 최근의 내시경기구와 기술의 발전으로 인하여 이러한 팽대부 종양이 조기에 발견된다면 내시경적으로도 치료할 수 있게 되었는데, 따라서 팽대부 선종의 조기발견의 중요성이 한층 강조되게 되었다. 저자들은 무증상 성인에게 있어서 십이지장 팽대부에서 조기에 우연히 발견된 관상융모상선종 1예를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다.
