Usefulness of [18F]-Deoxyglucose Positron Emission Tomography in Diagnosing the Patients with Corticobasal Degeneration

dc.description.abstractBackgrounds and OBJECTIVES: The corticobasal degeneration (CBD) is a neurodegenerative disease showing the progressive asymmetric extrapyramidal motor deficits, apraxia and cortical sensory loss. The asymmetric fronto-parietal cortical and subcortical hypometabolism was demonstrated in the previous [18F]-deoxyglucose positron emission tomography (FDG PET) scan studies. We investigated the hypometabolic pattern of Korean CBD patients using quantitative FDG PET and voxel based comparison method.
METHODS: Twelve patients with CBD and 16 age-matched controls were included in this study. Quantitative brain FDG PET and MRI scan studies were performed simultaneously in all subjects. For better spatial normalization of PET image, individual MR image was used for the normalization template. Using the group comparison model in the SPM2 software, we compared the local glucose metabolism between the CBD patients and normal controls.
RESULTS: The global glucose metabolic activity of the CBD patients was significantly lower than that of controls. In SPM analysis, the CBD patients showed significantly decreased glucose metabolism in the fronto-parieto-temporal cortices, thalamus and caudate nucleus. The contralateral hemisphere to the more affected side showed decreased glucose metabolism in much wider fronto-parieto-temporal cortices.
CONCLUSIONS: Quantitative FDG PET analysis showed asymmetric fronto-parieto-temporal hypometabolism in patients with CBD and is useful method in diagnosing the CBD.

dc.description.abstract서론: 피질기저핵변성은 좌우 비대칭의 추체외로 증상과, 실행불능증, 피질성 감각 소실을 특징으로 하는 진행성 신경계퇴행성 질환이다. 지금까지 [18F]- deoxyglucose 양전자방출단층촬영 (FDG PET) 연구에서 뇌 포도당 대사량이 전두엽과 두정엽에 걸쳐 비대칭적으로 감소되어 있는 것으로 알려져 있다. 본 연구에서는 정량적 FDG PET에서 나타난 한국인 피질기저핵변성 환자들의 뇌 포도당 대사량 패턴을 화소 기반 통계적 모수지도 기법 (SPM)을 이용하여 분석하고, 임상적 유용성에 대하여 평가하고자 했다.
방법: 12명의 피질기적핵변성 환자와 나이를 일치시킨 16명의 정상인을 대상으로 정량적 뇌 FDG PET과 뇌 자기공명영상을 동일한 날에 시행했다. FDG PET 영상의 공간적 정규화 정확성을 높이고자 피검자 개개인의 뇌자기공명영상을 정규화를 위한 기본틀로 이용했다. SPM2 소프트웨어의 군간 비교 통계를 이용하여 피질기저핵변성 환자들과 정상인의 국소 뇌 포도당 대사량의 차이를 비교했다.
결과: 피질기저핵변성 환자군의 전체 뇌 대사량은 정상인에 비해 현저하게 떨어져 있었다. SPM 분석에서 환자군은 전두엽, 두정엽, 측두엽에 걸쳐 뇌 피질 대사량이 떨어져 있었고, 시상과 꼬리핵의 대사량도 정상에 비해 떨어져 있었다. 증상이 심한 쪽의 반대편 대뇌 피질이 동측의 대뇌 피질에 비해 더 넓은 영역에서 포도당 대사량이 떨어져 있었다.
결론: 정량적 FDG PET을 이용하여 피질기저핵변성 환자에서 전두엽, 두정엽, 측두엽에 걸친 비대칭적인 뇌 포도당 대사량 감소를 확인할 수 있었으며, 이러한 뇌 대사량 패턴은 피질기저핵변성을 진단하는 데에 도움이 될 수 있을 것으로 생각된다.
