Prognosis of Transitional Cell Carcinoma of the Bladder in Patients 40 Years Old or Less : Comparison with Elderly Patients

Abstract
PURPOSE: Transitional cell carcinoma (TCC) of the bladder in young patients is rare with 0.4-2% of such tumors presenting in the first 4 decades of life. The natural history and prognosis of TCC of the bladder in young patients remain a matter of debate. This study aimed to evaluate whether TCC of the bladder in young patients is biologically different from elderly patients. MATERIALS AND METHODS: The medical records of 21 patients 40 years old or younger with bladder TCC treated in our institution between July 1995 and May 2005 (group 1) were retrospectively reviewed. For purposes of comparison, the medical records of 94 patients 51 years and older with bladder TCC treated in our institution during same period (group 2) were also reviewed. The clinicopathological variables and survival rates were assessed. RESULTS: Mean age at initial diagnosis was 36.1 years (25-40) and 67.3 years (51-92), and the male-to-female ratio was 2:1 and 3.7:1 in group 1 and 2, respectively. TCC of the bladder was diagnosed between ages 20 and 29 years in 2 patients (9.5%) and between ages 30 and 40 years in 19 (90.5%). Initial stage distribution was similar with 85.7% and 79.8% of patients presenting with superficial TCC in group 1 and 2, respectively. The gross hematuria as a presenting symptom (p=0.943), multiplicity of tumor mass (p=0.298), T stage (p=0.880), N stage (p=0.662), M stage (p=0.635), concomitant CIS (p=0.140), and grade (p=0.065) were not significantly different between the two groups. There was no difference in the cancer-specific survival rate between the two groups (p=0.905). In cases with superficial TCC, no difference was observed in the disease-free survival rate (p=0.821) or progression-free survival rate (p=0.595) between the two groups. CONCLUSIONS: TCC of the bladder in patients 40 years old or less has a similar clinicopathological distribution and disease outcome to those observed in elderly patients. It is suggested that the management and follow-up of TCC of the bladder in young patients should be the same as those for elderly patients.

목적: 방광 이행세포암이 젊은 연령에서 발생하는 경우는 드물어서, 모든 방광 요로상피종양의 0.4-2%가 40세 이전에 발생한다. 방광 이행세포암이 젊은 연령에서 발생하는 경우에 고연령층에서 발생하는 경우에 비해 예후가 더 좋다는 보고들도 있는 반면에 예후에 차이를 보이지 않는다는 보고들도 있어 논란의 여지가 있다. 이에 저자는 방광 이행세포암 진단 당시 나이가 40세 이하인 젊은 연령의 환자들과 51세 이상인 고연령 환자들 사이에 임상병리학적 분포와 및 예후에 있어서 차이가 있는지를 알아보고자 하였다. 대상 및 방법: 1995년 7월부터 2005년 5월까지 본원 비뇨기과에서 방광 이행세포암으로 치료 받은 40세 이하의 환자들 (1군) 21명을 대상으로 후향적으로 조사하였다. 비교를 하기 위하여 동일한 기간 동안에 본원 비뇨기과에서 방광 이행세포암으로 치료 받은 51세 이상인 환자들 (2군) 94명에 대해서도 조사하였다. 두 군 사이에 임상병리학적 변수들과 생존율을 비교 분석하였다. 결과: 진단 당시 평균 나이는 1군이 36.1세 (25-40), 2군이 67.3세 (51-92)였고, 남녀비는 1군이 2:1, 2군이 3.7:1이었다. 1군 21명 중 2명 (9.5%)에서는 20-29세에 방광 이행세포암이 진단되었고, 19명 (90.5%)에서는 30-40세에 방광 이행세포암이 진단되었다. 진단 당시 1군에서는 85.7%가, 2군에서는 79.8%가 표재성 암을 보였다. 내원 당시 주증상으로서 육안적 혈뇨 (p=0.943), 방광암의 다발성 유무 (p=0.298), T 병기 (p=0.880), N 병기 (p=0.662), M 병기 (p=0.635), 상피내암 동반 유무 (p=0.140) 및 분화도 (p=0.065)에서 두 군 사이에 통계학적으로 유의한 차이가 없었다. 두 군 사이에 암특이 생존율 (p=0.905)은 유의한 차이가 없었고, 표재성 암 환자들만을 대상으로 무병 생존율 (p=0.821) 및 무진행 생존율 (p=0.595)을 비교해 보았을 때에도 유의한 차이가 없었다. 결론: 방광 이행세포암이 40세 이하에서 발생한 경우에 51세 이상의 고연령층에서 발생한 경우와 비교하여 임상병리학적 분포와 예후에 있어서 차이가 없었다. 따라서 방광 이행세포암이 40세 이하의 젊은 연령에서 발생한 경우에도 고연령층에서 발생한 경우와 동일한 치료 및 추적관찰을 권장한다.
