| dc.description.abstract | "목적: 최근 당뇨병은 지속적으로 증가하고 있으며 이에 동반되는 합병증들에 의해 건강상의 위험에 처할 수 있는 문제점을 나타내고 있다. 보다 철저한 혈당 관리가 유의하게 당뇨병의 만성 합병증의 발생율을 낮춘다는 연구 결과는 당뇨병을 진단받은 환자 중에서 만성 합병증의 발생 가능성이 높은 군을 선별하여 철저히 관리하는 것이 도움이 될 수 있음을 보여준다. 당뇨병 환자들에게 있어서 혈관 질환의 발생과정에 관여하는 것으로 판단되고 있는 효소들의 단백뇨의 정도에 따른 변화 여부를 조사하고 이를 통해서 당뇨병의 합병증이 발생할 가능성이 있는 사람을 조기에 선별할 수 있는 지표로서 의미가 있는지 보고자 한다.
방법: 아주대학교 병원 내분비대사내과에서 추적 관찰중인 38명의 제2형 당뇨병 환자 중 미세 단백뇨 군 10명, 현성 단백뇨 군 8명, 나이와 성별을 고려한 비 단백뇨 군 20명으로 대상으로 병력 청취, 신체 계측을 시행하고 ELISA법으로 혈청 MMP-2, MMP-9, TIMP-1 농도를 측정하였다. 결과: 혈청 MMP-2, MMP-9, TIMP-1 농도를 측정한 결과 MMP-2와 MMP-9의 농도는 각 군에서 차이를 보이지 않았으며 TIMP-1은 단백뇨가 많이 배설되는 군 일수록 통계학적으로 유의하게 혈청 농도가 증가되는 결과를 나타내었다. 결론: 당뇨병성 신증의 특징인 세포 외 기질의 분해 감소 및 침착 증가가 일어나는 과정에서 TIMP-1이 영향을 미칠 것으로 보인다.
