dc.description.abstract"배경: 비갑상선질환 증후군은 triiodothyronine (T3)치가 단독으로 저하되어 있거나 T3 와 tyroxine (T4)치가 같이 저하되어있는 것이 특징이다. 이는 기아, 패혈증, 심근경색, 심장 혹은 비 심장수술 환자에서 관찰 되며, 어떤 심한 질병과도 동반될 수 있다. 원인은 명확히 알려져 있지 않으나, interleukine-6 (IL-6), tumor necrosis factor-a (TNF-a)와 같은 cytokines가 요소 중 하나로 제시되고 있다. 간이식수술은 cytokines의 급격한 변화를 겪는 수술인데, 수술 중 혹은 수술 후 갑상선 호르몬 변화에 대한 보고는 거의 없다.

대상 및 방법: 간이식수술을 시행 받는 20명의 환자를 대상으로 하였다. 수술 전 내분비 질환이 있는 환자는 제외하였으며, 마취 유도후, 무간기, 수술이 끝나기 30분전, 수술 24시간 후, 수술 120시간 후 총 5회에 걸쳐 혈중 T3, T4, free-triiodothyronine(FT3), free-tyroxine(FT4) thyroid stimulating hormone(TSH) 농도를 측정하였다. Cytokines들 중 IL-6의 값도 같은 시점에 측정하였다.

결과: 측정한 갑상선 호르몬 중 혈중 T3, T4, TSH 농도는 마취유도후 시행한 결과보다 무간기, 수술 끝나기 30분전, 수술 24시간 후, 수술 120시간 후에 유의하게 감소하였다. 특히 혈중 T3 농도는 무간기에 시행한 결과에 비해서도 수술 24시간 후, 수술 120시간 후에 유의하게 감소하였다.

FT3 농도는 마취 유도후 결과보다 수술 120시간 후에 유의하게 감소하였다. FT4 농도는 마취 유도후 결과에 비해 수술 24 시간 후, 수술 120 시간 후에 유의하게 감소되었다.

IL-6의 값은 마취유도후 결과보다 무간기, 수술 끝나기 30분전 결과가 유의하게 증가하였으며, 이후 다시 감소하였다.

결론: 간이식 수술을 받는 환자는 비갑상선질환 증후군을 겪는다고 생각되어지며, T3 등의 갑상선 호르몬 감소는 IL-6의 농도와 역비례하게 상관관계가 있는 것은 아니었다. 비갑상선질환 증후군의 발생 원인 및 갑상선 호르몬의 투여 여부는 논란의 여지가 남아 있으므로, 이에 대한 추가적인 연구가 필요 할 것으로 생각된다."
