Prognostic Value of Bone Marrow Micrometastasis Detected by Nnested RT-PCR for Cytokeratin 19 and Mammaglobin in Breast Cancer

Abstract
BACKGROUND/AIMS: Breast cancers in the early phase frequently undergo distant metastasis, and survival of patients is greatly dependent on distant metastasis. Occurrence of micrometastasis has been suggested to relate with prognostic features of breast cancer, such as lymph node metastasis and the presence of vascular invasion. The aim of this study was to examine the presence of keratin-19 and mammaglobin mRNA of epithelial tumors in bone marrow aspirates obtained from breast cancer patients and its possible correlation with tumor staging and disease free survival.
METHODS: Bone marrow samples were obtained from 250 breast cancer patients at the time of surgery. We separated the mononuclear fraction from the samples and carried out nested reverse transcriptase polymerase chain reaction (RT-PCR) for the detection of keratin-19 and mammaglobin mRNA with two different pairs of primer. After operation, the patients were followed up by 3-month intervals. We studied possible correlation with tumor size, nodal involvement, stage, and distant metastasis.
RESULTS: Seventy-five out of 250samples were studied cytokeratin 19 and others were studied cytokeratin and mammaglobin. Median follow-up time was 21.1 months. Sixty-five(26%) out of 250 samples were cytokeratin 19 positive and 25 (14.3%) out of 175 were mammaglobin positive. Eight cases (12.3%) in cytokeratin positive group showed recurrent legions in the distant organ. Whereas six (3.2%) out of 185 cytokeratin negative patients had distant recurrences. Mammaglobin was not correlated with distant recurrences. Bone marrow micrometastasis, stage, axillary nodal status, and estrogen receptor were independent.
CONCLUSIONS: Bone marrow micrometastasis detected by nested RT-PCR for cytokeratin 19 can be a useful predictive marker for distant recurrence of breast cancer.

목적: 유방암은 비교적 원격전이가 잘되는 고형 종양이며 환자의 생존율은 원격전이의 여부에 의하여 결정이 된다. 따라서 타 장기의 원격전이를 조기에 발견하고자 하는 노력이 계속 되어 왔으며 골수의 미세전이 측정도 그 중의 하나라고 할 수 있다. 골수 전이의 표지자로는 cytokeratin과 mammaglobin이 널리 이용되고 있으며 이에 대한 유용성은 아직도 확립되고 있지 않다. 이에 저자들은 역전사 중합효소반응법을 이용하여 골수에서 cytokeratin과 mammaglobin을 측정하여 골수 미세 전이가 원격전이에 미치는 영향을 연구하였다.
대상 및 방법: 2001년 6월부터 2003년 12월까지 본원에서 유방암으로 수술 받은 환자 중 골수를 채취한 254명에 대해서 후향적으로 분석하였다. 여성이 252명, 남성이 2명이었으며 나이는 25세에서 80세까지로 평균나이는 46세였다. 수술 전 전신마취 하에서 골수 천자를 시행하였으며 유핵 세포를 분리한 후 RNA를 추출하여 cytokeratin 19과 mammaglobin에 대해 역전사 중합 효소 반응법을 시행 하였다. 대조군으로는 β-actin을 이용하였으며 골수 미세 전이와 기존 예후인자 및 재발과의 관계를 chi square법을 이용하여 분석하였으며 무전이 생존율에 대해 Kaplan-Meier법을 이용하여 log rank법으로 분석하였다.
결과: 환자의 병기는 관상피내암이 16명, 1기 58명, 2기 126명, 3기 43명, 4기 4명이었으며 cytokeratin 19만 측정한 군이 76명 두 가지 모두 측정한 군이 178명 이었다. 골수에서 cytokeratin 19에 양성은 66명(26%)이었으며 mammaglobin에 양성은27명(15.2%), 두 가지 모두 양성인 군은 11명이었다. 총 19예의 재발이 관찰 되었으며 원격전이는 14예였다. 골수 미세 전이가 없던 군에서 6명, cytokeratin 19 양성 군에서 8명, mammaglobin 양성 군에서 1명의 원격 전이가 관찰되어 cytokeratin 19 양성 군에서 의미 있게 많은 원격 전이가 관찰되었다(p=0.019). cytokeratin 19과 mammaglobin의 발현 여부와 병기, 호르몬 수용체, 조직학적 등급 및 c-erb B2 간에 유의한 상관성은 관찰되지 않았다. 무전이 생존율에 있어서도 cytokeratin 19 양성 군에서 유의하게 낮았으며(p=0.01) mammaglobin은 통계적 차이를 보이지 않았다.
결론: 짧은 추적 관찰 기간에도 cytokeratin 19 양성군에서 많은 원격 전이와 낮은 무전이 생존율이 관찰되어 의미 있는 표지자 임을 확인 하였으며 예후인자로서의 가치가 있을 것으로 생각된다. 골수에서 cytokeratin 19의 발현과 기존의 예후 인자와는 상관 관계가 없어 골수 미세 전이가 독립적인 예후 인자로써의 가능성이 있을 것으로 생각 되며 추적 관찰 기간이 연장 되고 재발의 예가 증가 한다면 더욱 확실해 질 것으로 생각한다.
