dc.description.abstract연구 배경: 대사증후군의 진단 기준은 현재 National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) 의 기준이 세계적으로 받아들여지고 있다. 그러나, 서양인의 체형에 근거한 이러한 진단기준은 동양인에게 부적합한 것으로 보고되고 있다. 2000년 WHO 서태평양 지부에서는 동양인의 비만의 진단 기준으로 체질량 지수는 25.0 kg/m2 이상, 복부비만의 허리 둘레의 기준치는 남자 90 ㎝, 여자 80 ㎝ 이상으로 추천하였다. 이 논문의 목적은 한국인 성인에서 대사증후군을 진단하기 위한 적절한 허리둘레의 기준치를 분석하고자 하였다. 방법: 2001년 4월부터 6월까지 한국건강관리협회 검진센터에 내원한 30세에서 60세 사이의 한국인 성인 824명 (남성 427명, 여성 397명)을 대상으로 하였다. 대사증후군은 2001년 NCEP ATP III 기준을 용하였고 ① 고중성지방혈증 (=150 mg/dL) ② 저 HDL 콜레스테롤혈증 (남자 < 40 mg/dL, 여자 <50 mg/dL) ③ 고혈압 (혈압강하제를 복용하고 있거나, 수축기혈압 = 130 mmHg 또는 이완기혈압 = 85 mmHg) ④ 고혈당 (혈당강하제를 복용중이거나 공복혈당 =110 mg/dL) 의 기준 중 3가지 이상을 갖고 있는 경우를 대사증후군으로 정의하였다. 결과: 총 대상군의 21.7%에서 대사증후군이 동반되어 있었다. ROC 분석에 따라 대사증후군을 증가시키는 체질량지수의 cut-off value는 남성에서 24.8 kg/m2 (민감도 63%, 특이도 61%, p<0.001) 였고 여성에서 24.9 kg/m2 (민감도 70%, 특이도 61%, p<0.001) 이었다. 같은 방법으로 허리둘레의 cut-off value는 남성에서 86 cm (민감도 65%, 특이도 54%, p<0.001)였고 여성에서 82 cm (민감도 71%, 특이도 60%, p<0.001)이었다. 결론: 한국인에서 대사증후군을 증가시키는 체질량지수의 기준치는 25 kg/m2이며, 이는 2000년 WHO의 서태평양 지회의 비만 진단기준과 잘 일치한다. 또한 한국인에서 대사증후군을 진단하기 위한 허리둘레의 기준치는 대사증후군의 유병률을 증가시키는 민감도와 특이도를 고려했을 때, 남성은 90cm, 여성은 85cm가 타당할 것으로 생각된다.-
dc.description.abstractBackgrounds: The metabolic syndrome is the clustering of cardiovascular risk factors such as glucose intolerance, hypertension, dyslipidemia and visceral obesity. The National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) criterion for the diagnosis of metabolic syndrome is now widely used in many parts of the world. However, applying this criteria to an Asian population, could underestimate the prevalence of the metabolic syndrome. Especially anthropometric index such as waist circumference is inappropriate for Asian population. In 2000, the WHO Western Pacific Region recommended the cut-off value for obesity in Asians to body mass index (BMI) over 25.0 kg/m2 and abdominal obesity to waist circumference (WC) over 90 cm in men and 80 cm in women. But, among Asians, each ethnic group has their own anthropometric character, so it is difficult to apply the same criteria to different ethnic group in Asia. Thus, the aim of this study is to validate the appropriate cut-off value of WC to increase the risk of the metabolic syndrome in Korean adult population. Methods: A total of 824 subjects (427 men, 397 women), aged 30~60 years (average 47.87.8 years), who underwent routine medical check-up in Korea Association of Health Promotion, were included in this study. The metabolic syndrome was defined as patients with three or more of the followings: Hypertriglyceridemia (=150 mg/dL) low HDL-cholestetol (Male < 40 mg/dL, Female < 50 mg/dL) Hypertension (Taking antihypertensive medication or systolic blood pressure = 130 mmHg or diastolic blood pressure = 85 mmHg) Glucose intolerance (Taking oral hypoglycemic agent or fasting glucose =110 mg/dL). Results: The metabolic syndrome was seen in 21.7% of total subjects; glucose intolerance in 18.2%, hypertension in 56.4%, hypertriglyceridemia in 44.8%, and low HDL-cholesterolemia in 42.0%. With metabolic score, who had 0 components of metabolic syndrome in 16.7%, 2 components in 30.9%, 3 components in 18.0%, and all 4 components in 3.8 % of our enrolled subjects. The cut-off value of BMI to correspond with the metabolic syndrome by ROC curve was 24.8 (sensitivity 63%, specificity 61%, p<0.001) in men and 24.9 kg/m2 (70%, 61%, p<0.001) in women. The cut-off value of WC was 86 cm in men (65, 54%, p<0.001) and 82 cm in women (71, 60%, p<0.001). When the subjects were divided into 5 groups according to the WC, the odds ratio for the metabolic syndrome was 5.17 (95% CI 2.11-12.67) in men and 16.94 (2.17-132.41) in women with the cut-off value of 90 cm in men and 85 cm in women. Conclusion: We concluded that WHO Western Pacific Region recommendation guideline as a cutoff value of BMI for obesity in Korean adult was reasonable, but not of WC. Therefore, we recommended the modified cut-off value of WC for abdominal obesity in Korean adult to 90 cm in men and 85 cm in women.-
