Bilateral Disc Swelling associated with Posterior Scleritis in a Child

PURPOSE: To report a case of an unusual presentation of posterior scleritis in a child.
METHOD: A 7-year-old boy suffering from headache and binocular pain of 3 weeks duration was referred to our hospital with the diagnosis of optic neuritis. Complete ophthalmic and systemic examinations including ocular ultrasonography, magnetic resonance imaging (MRI), and laboratory testing were carried out and the results were evaluated.
RESULT: On examination he had severe bilateral optic disc swelling and the elevation of macula. Diffuse bilateral posterior scleral thickening and fluid in sub-Tenon’s space were found in B-scan ultrasonography. Laboratory findings and MRI disclosed no associated ocular or systemic disease. Systemic steroid treatment got his inflammatory condition under control.
CONCLUSION: This case illustrates an interesting presentation of posterior scleritis in a 7-year-old and this condition should be recognized in children as part of the differential diagnosis of acute ocular inflammatory process.
