Association between Depression and Cardiovascular Risk Factors in Workers Undergoing Comprehensive Medical Examination in a University-based Hospital

Abstract
Background: Depression increases the risk of cardiovascular risk factors in adults. However, the association between depression and cardiovascular risk factors was not well evaluated in South Korea. Our study was done to evaluate the association between depression and cardiovascular risk factors in workers.



Methods: The study population was selected among adults working in several industries that had more than 50 employees. They had received their medical examinations at the Health Promotion Center of Hallym University Sacred Heart Hospital between March and November 2005. We estimated the odds ratio to determine whether depression (Korean Center for Epidemiologic Studies Depression Scale score ≥21) was associated with each cardiovascular risk factors (hypertension, diabetes mellitus, hypercholesterolemia, and obesity).



Results: The prevalence of depression was 15.5%. Depression was significantly associated with hypertension (P<0.001), obesity (P=0.03) and hypercholesterolemia (P=0.004). The adjusted odds ratio for hypertension and hypercholesterolemia were 1.41 (95% CI 1.04∼1.91) and 1.59 (95% CI 1.05∼2.40), respectively. The association between depression and obesity was eliminated after adjustment (1.25, 95% CI: 0.97∼1.61).



Conclusion: Depression was significantly associated with hypertension and hypercholesterolemia in workers. Further long-term study on the association of depression with cardiovascular risk factors is necessary.

연구배경: 성인에서 우울증이 심혈관질환 위험도 및 심혈관질환 위험요소들의 위험도를 증가시킨다는 외국의 연구들이 있었다. 그러나 우리나라에서는 이에 대한 연구가 별로 없었다. 본 연구는 직장인의 우울증과 각각의 심혈관질환위험요인의 관계에 관하여 평가하고자 하였다.



방법: 2005년 3월 1일부터 11월 30일 까지 한림대학교병원 건강검진센터에서 건강검진을 시행한 직장인 2,095명을 대상으로 자기기입식 설문조사를 시행하여 우울증과 심혈관질환 위험요인과의 관련성을 분석하였다. Center for epidemiologic studies depression scale (CES-D) 점수 21점을 기준으로 비우울증군과 우울증군으로 나누어 독립변수로 두고 고혈압, 당뇨, 고지혈증, 비만을 종속변수로 두어 우울증이 심혈관질환 위험요인을 높이는지 여부 대해 다중 로지스틱 회귀 분석을 시행하였다.



결과: 우울증의 유병률은 15.5% (324명)이었다. 단변량 분석 결과 고혈압과 고지혈증, 비만이 우울증과 관련이 있었다. 고혈압은 우울증군에서 25.2%로 비우울증군의 16.1%에 비해 유의하게 높았다(P<0.001). 고지혈증은 우울증군에서 11.1%로 비우울증군의 6.6%에 비해 유의하게 높았다(P=0.004). 비만의 경우 우울증군에서 39.5%로 비우울증군의 33.1%에 비해 유의하게 높았다(P=0.026). 당뇨의 경우 우울증군에서 6.0%로 우울증군의 3.7%로 높았으나 유의하지 않았다(P=0.06). 다중 로지스틱 회귀 분석 결과 우울증 여부는 고혈압과 고지혈증과 유의한 관계를 보였으며, 교차위험비는 각각 1.41 (95% CI 1.04∼1.91), 1.59 (95% CI 1.05∼2.40)이었다.



결론: 우울증군이 비우울증군에 비해 고혈압과 고지혈증이 유의하게 높았으며 당뇨와 비만과는 관련성이 없었다. 본 연구는 단면조사 연구로 우울증과 심혈관질환 위험요인과의 인과관계를 밝힐 수 없었다. 이를 위해 우울증과 심혈관질환 위험요인간의 인과관계에 관한 전향적 연구가 필요할 것으로 생각된다.
