dc.description.abstractThe purpose of the study was to determine the association of depression and anxiety in stroke patients with daily life activities and rehabilitation efforts and to provide basic information needed to promote physical activities and rehabilitation success among stroke patients. A total of 99 stroke patients in 5 university hospitals and 1 rehabilitation center based in Suwon, Gyeonggi Province and Seoul enrolled in the study. And data were collected during the period from August 2006 to September 2006. The Modified Barthel Index, developed by Shah et al.(1989) to evaluate daily life activities of stroke patients, was used in this study. A Beck Depression Inventory (BDI), developed by Beck et al.(1961) and modified and translated into Korean by Han Heung-Moo(1986), was used to measure depression levels. A Beck Anxiety Inventory (BAI), developed by Beck et al.(1988) and modified and translated into Korean by Yuk Sung mu(1996), was used to measure anxiety levels. A SPSS PC 12.0 was employed for statistical analysis, including descriptive statistics, t-test, one-way ANOVA, Pearson's correlation and multivariate regression.
1. Daily activity scores ranged from 2 to 100 out of a total score of 63 with the mean sore of 71.47(±28.52). Anxiety scores ranged from 0 to 45 out of a total score of 63 with the mean score of 14.4(±9.91). Depression scores ranged from 1 to 50 with out of a total score of 63 with the mean score of 20.0(±9.95).
2. Pearson's correlation analysis revealed a negative correlation between daily life activities and anxiety (r = -0.35, p = 0.00) and depression (r = -0.27, p = 0.07). But there was a significant positive correlation between levels of depression and anxiety (r = 0.55, p = 0.00).
3. Multivariate linear regression analysis was performed to finding the relationships among variables found significant in t-test, one-way ANOVA and Pearson's correlation analysis. Those respondents living with their family(B = -11.78, p = 0.01) showed a negative correlation between physical activity level in daily life (B = -0.09, p = 0.02) and levels of anxiety. There were negative correlations between educational levels(junior high school graduates :B = -7.56, p = 0.02, college graduates :B = -9.73, p = 0.00) and depression levels and between physical activity level in daily life(B = -0.07, p = 0.04)and depression levels.
Thus the study provided evidence that depression and anxiety in stroke patients significantly affect their daily life activities and that there is a significantly positive correlation between levels of depression and anxiety. Many stroke patients slip into depression and anxiety as they have difficulty performing daily functions, and levels of depression and anxiety are correlated with their physical activity levels. It is therefore necessary to provide stroke patients with emotional support as part of the rehabilitation programs to help them increase rehabilitation efforts and quality of life.

dc.description.abstract뇌졸중 환자들의 재활에 있어서 꼭 필요한 재활운동에 대한 의욕 혹은 재활동기를 저해하는 심리적인 문제들 중에서 우울과 불안이 일상생활동작 수행능력과 어떠한 관련성이 있는지 알아봄으로서 뇌졸중 환자들의 재활동기를 증가시키기는 중재프로그램의 개발을 위한 기초자료를 제공하고자 본 연구를 시행하였다. 연구대상은 서울과 경기도 수원시에 소재하고 있는 종합병원 5곳과 재활병원 1곳에서 치료를 받고 있는 뇌졸중 환자 99명을 대상으로 하였으며, 자료수집기간은 2006년 8월부터 9월까지 2개월 간 실시하였다. 연구도구로는 일상생활동작 수행을 평가하기 위하여 Shah 등(1989)이 뇌졸중 환자를 위해 개발한 MBI(Modified Barthel Index)를 사용하였고, 우울의 정도를 평가하기 위한 도구로서 Beck 등(1961)이 개발하고 한흥무(1986)에 의해 한국어로 표준화된 척도인 BDI(Beck Depression Inventory)을 사용하였으며, 불안의 정도를 평가하기 위한 도구로서 Beck 등(1988)이 개발하고 육성필(1996)에 의해 한국어로 표준화된 척도인 BAI (Beck Anxiety Inventory)을 사용하였다. 자료의 분석방법은 SPSS PC 12.0을 이용하였으며, 기술적인 통계, t-검정, 일요인 분산분석 및 다중회귀분석 등의 통계분석을 실시하였다.
1. 일상생활동작의 점수는 총점이 100점 만점이고 최소 2점에서 최대 100점까지 분포하였으며, 평균은 71.47(±28.52)이었다. 불안의 점수는 총점이 63점 만점이고 최소 0점에서 최대 45점까지 분포하였으며, 평균은 14.4(±9.91)이었다. 우울의 점수는 최소 1점에서 최대 50점까지 분포하였으며, 평균은 20.0(±9.95)이었다.
2. 피어슨 상관관계를 이용한 분석결과 불안(r = -0.35, p= 0.00), 우울(r = -0.27, p=0.07)은 일상생활동작 수행능력과 각각 유의한 음의 상관관계를 보였고, 우울은 불안(r=0.55, p=0.00)과 유의한 양의 상관관계를 보였다.
3. t-검정과 일요인 분산분석 그리고 상관관계분석의 결과를 바탕으로 유의하게 나온 변수들에 대한 다중회귀분석을 실시한 결과, 배우자 및 자녀와 함께 거주하는 경우(B = -11.78, p = 0.01)에, 일상생활동작 수행능력이 높을수록(B = -0.09, p = 0.02) 불안 점수가 낮았다. 우울점수는 중졸미만에 비해 교육수준이 증가할수록(중졸:B = -7.56, p = 0.02, 대졸:B = -9.73, p = 0.00) 그리고 일상생활동작 수행능력이 높을수록(B = -0.07, p = 0.04) 우울 점수가 낮았다.
따라서 일상생활동작 수행능력은 우울과 불안에 각각 유의한 관련성이 있으며 불안과 우울상호간에는 높은 관련성이 있음을 알 수 있었다. 뇌졸중 이후에 오는 불안과 우울은 동반되어 증상이 나타나는 경우가 많으며, 특히 뇌졸중으로 인해 일상생활동작 수행능력이 낮을수록 불안과 우울 발생률이 높은 것으로 판단된다. 그러므로 뇌졸중 환자에 대한 불안과 우울예방을 위한 정서적 지원 등의 재활프로그램에 비중 있게 포함됨으로써 재활동기, 삶의 질 개선 향상에 기여할 것으로 생각된다.
