Menopausal symptoms and quality of life between chemotherapy-induced menopause women with breast cancer and natural menopause women without cancer.

Abstract

This study investigate a menopausal symptoms and quality of life of chemotherapy-induced menopause women with breast cancer, and compared with natural menopause women without cancer. The participants were recruited from the Breast Cancer Center at K hospital in Seoul. From September 4, 2017 to June 27, 2018, a total of one hundred twenty eight women were included, 64 with chemotherapy-induced menopause women with breast cancer and 64 with natural menopause women without cancer. The menopausal symptoms was measured by the Korean version of the Menopause Rating scale (MRS) developed by Heinemann in 2004. The quality of life was measured by the Short-Form 36 Questionnaire Version 2 (SF-36V2), which is the international version of SF-36 developed by RAND(Research ANd Development) corporation. The collected data was analyzed by descriptive statistics, Mann-Whitney U test, χ² test and Independent T test. The results of this study could be summarized as follows. 1. The mean age of chemotherapy-induced menopause women with breast cancer was 49.1(±5.4) years, whereas the mean age of natural menopause women without cancer was 52.4(±5.5) years. Age at menopause was 46.5(±5.5) years and 49.6(±4.4) years for women with and without breast cancer, respectively. The differences between the two groups were not significant in other categories such as period of menopause, level of education, existence of occupation and spouse, health status, and satisfaction of life. 2. A mean score of MRS with chemotherapy-induced menopause women with breast cancer was 17.8(±8.2), higher than a mean score 12.9(±8.4) for natural women without cancer. There were significant differences in total MRS score between chemotherapy-induced menopause women with breast cancer and natural menopause women without cancer with respect to ‘psychological symptoms’, ‘somato-vegetative symptoms’ and ‘urogenital symptoms’. The sequential order of menopausal symptoms for chemotherapy-induced menopause women with breast cancer was ‘exhausted’, ‘sweating/hot flashes’, and ‘joint & muscle complaints’. The sequential order of menopause symptoms for women without cancer was ‘exhausted’, ‘joint & muscle complaints’, ‘sleeping disorders’, ‘irritable’ and ‘sweating/hot flashes’. 3. The chemotherapy-induced menopause women with breast cancer experienced more menopausal symptoms in ‘sweating/hot flashes’, ‘sexual problems’, and ‘vaginal dryness’ than the menopause women without cancer. Breast cancer women with chemotherapy-induced menopause experienced severe menopausal symptoms in ‘exhausted’, ‘sweating/hot flashes’, ‘vaginal dryness’, ‘sleeping disorders’, ‘urinary complaints’, and ‘depressed’ as compared with natural menopause women 4. There were significant differences between chemotherapy-induced menopause women with breast cancer and natural women without cancer in quality of life: ‘role-physical’, ‘role-emotional’ and ‘general health’. In conclusion, chemotherapy-induced menopause women with breast cancer are exposed to a variety of severe menopausal symptoms than menopause women without cancer, resulting in decrease of quality of life. Therefore, it is suggested that the development of differentiated guidelines focused on symptoms is necessary, which should be provided continuously not only before treatment but also after treatment is complete. Increase in the number of specialists is also necessary to manage them professionally.

본 연구는 화학요법관련 인공폐경 유방암 여성의 갱년기 증상과 삶의 질을 파악하고 자연폐경 여성과 비교하여 인공폐경 유방암 여성을 위한 간호중재 개발을 위한 기초자료를 마련하려는 목적으로 시도된 서술적 조사연구이다. 본 연구의 2017년 9월 4일부터 2018년 6월 27일까지 서울 소재의 3차 의료기관인 K병원의 유방암 센터를 방문한 화학요법관련 인공폐경을 경험한 여성 64명과, 자연폐경 여성 64명, 총 128명을 대상으로 진행하였다. 자료수집 방법은 Heinemann 등이 2004년 개발한 갱년기 증상 측정도구 Menopause Rating Scale (MRS)의 한국어판을 사용하였고, 삶의 질은 RAND corporation에 의해 개발된 SF-36의 국제버전인 Short-Form 36 Questionnaire Version 2 (SF-36V2)인 자가보고형 설문지를 사용하여 수집하였다. 수집된 자료는 기술통계, Mann-Whitney U검정, χ²검정 및 Independent t-test로 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 화학요법관련 인공폐경 유방암 여성과 자연폐경의 평균나이는 인공폐경 유방암 여성은 49.1(±5.4)세로 자연폐경 여성의 52.4(±5.5)세 보다 낮았다. 평균 폐경나이는 인공폐경 유방암 여성은 46.5(±5.5)세, 자연폐경 여성은 49.6(±4.4)세로 인공폐경 유방암 여성이 더 낮았으며, 그 외 폐경기간, 교육수준, 직업유무, 배우자유무, 건상상태, 삶에 대한 만족도 등의 항목에서의 차이는 통계적으로 유의하지 않았다. 2. 화학요법관련 인공폐경 유방암 여성과 자연폐경 여성의 갱년기 증상 점수는 인공폐경 유방암 여성이 17.8(±8.2)점으로 자연폐경 여성의 12.9(±8.4)점보다 높았다. 갱년기 증상의 하위영역인 ‘비뇨생식기 증상’, ‘신체적 증상’, ‘정신적 증상’ 에서도 인공폐경 유방암 여성의 점수가 자연폐경 여성보다 높았다. 인공폐경 유방암 여성의 갱년기 증상 문항별 점수는 ‘신체 및 정신적 피로’, ‘안면 홍조, 발한’, ‘관절 및 근육 불편감’, ‘수면 문제’, ‘질 건조감’ 순이였으나, 자연폐경 여성은 ‘신체 및 정신적 피로’, ‘관절 및 근육 불편감’, ‘수면 문제’, ‘과민성’, ‘안면 홍조, 발한’ 순으로 나타났다. 3. 화학요법관련 인공폐경 유방암 여성이 자연폐경 여성보다 많이 호소하는 갱년기 증상은 ‘안면 홍조, 발한’, ‘성적인 문제’, ‘질 건조감’ 이였고, 심한 증상을 호소하는 갱년기 증상은 ‘신체 및 정신적 피로’, ‘안면 홍조, 발한’, ‘질 건 조감’, ‘수면 문제’, ‘배뇨 문제’, ‘우울한 기분’ 이였다. 4. 화학요법 관련 인공폐경 유방암 여성과 자연폐경 여성의 삶의 질 점수 중, ‘사회적 역할’, ‘정신적 역할’, ‘일반건강’ 영역에서의 차이는 통계적으로 유의하였다. 이상의 연구 결과를 종합해 볼 때, 화학요법관련 인공폐경을 경험한 유방암 여성은 자연폐경 여성보다 다양한 갱년기 증상에 노출되어 있으며, 이러한 증상은 삶의 질 저하에 영향을 미칠 수 있다. 따라서 증상에 초점을 둔 차별화된 가이드라인이나 지침 개발이 필요하며 이는 치료 전뿐만 아니라 치료가 종료된 후에도 지속적으로 제공되어야 하고, 이를 전문적으로 관리할 수 있는 전문 인력의 확충 또한 필요함을 제언하다.
